Our new office will remain in Lawrenceville and be in the
Terrace Park Medical Center Building located at 771 Old Norcross Road, Suite 260
(at the corner of Old Norcross Rd. and Lawrenceville-Suwanee Rd.).  Please check our directions page if your appointment is May 3rd or later.

When you come in to our office as a patient, we will ask you to complete a Patient Information form and a Dermatology Questionaire. Please bring these completed forms with you when you come to the office for your appointment.
